چکیده

The anhepatic phase of an orthotopic: liver transplantation procedure occun when the surgeon has removed the native liver (along with the intrahepatic: portion of the vena cava) and is involved in sewing in the donor organ. During this interval. the inferior vena cava is c:ross-clamped at the diaphragm and at a point just above the entry of the renal veins. The result is a complete interruption of venous return from the inferior vena cava. u is evident from a sudden decrease in both central venous and pulmonary arterial wedge pressures, resulting . in a profound reduction of cardiac: output. I The portal vein is also c:lamped. of necessity. during this phase. A sudden and marked inc:rase in hydrostatic: pressure in both the portal and systemic: venous beds thus occ:un. This not only causes damage to the kidneys, bowel. and pancreas. but also can profoundly exacerbate hemorrhage from the tissues that were cut during the recipient hepatectomy.

Severe Acute Hyperkalemia during Pre-Anhepatic Stage in Cadaveric Orthotopic Liver Transplantation

A serious hazard to patients during orthotopic liver transplantation is hyperkalemia. Although the most frequent and hazardous hyperkalemia occurs immediately after reperfusion of the newly transplanted liver, morbid hyperkalemia could happen in the other phases during orthotopic liver transplantation. However, pre-anhepatic hyperkalemia during orthotopic liver transplantation is rare. This rep...

Shunt flow and caval pressure gradient during veno-venous bypass in human orthotopic liver transplantation.

The routine use of veno-venous bypass during the anhepatic phase of orthotopic liver transplantation is controversial. Decreased shunt flows (1.5-3.0 litre min-1), as reported in the literature, may explain the lack of beneficial effects on outcome. We have studied the influence of bypass flows on caval pressure gradient (CPG) and renal perfusion pressure (RPP) in 45 patients undergoing orthoto...

Fatal haemothorax following large-bore percutaneous cannulation before liver transplantation.

Percutaneous bypass catheters are routinely used for veno-venous bypass (VVBP) during orthotopic liver transplantation (OLT). The recognized risks include bleeding, injury of vascular and nerve structures and lymphatic leakage. We describe a case where there were difficulties during catheterization and the patient suffered a cardiac arrest on commencing VVBP. Post-mortem examination revealed th...

The Effect of Intraoperative Restricted Normal Saline during Orthotopic Liver Transplantation on Amount of Administered Sodium Bicarbonate

Background: Severe metabolic acidosis occurs during orthotopic liver transplantation (OLT) particularly during the anhepatic phase. Although NaHCO3 is considered as the current standard therapy, there are numerous adverse effects. The aim of this study was to determine whether the restricted use of normal saline during anesthesia could reduce the need for NaHCO3.Methods: In this study we enroll...
